Published Authors
Soon to be Published Authors should Complete (1) the Author Contract and Agreement Form and (2) the Video Consent and Release Form, as described below.
Purpose of Author Contract and Agreement
Located at the top of the Author Contract is a list of specified responsibilities which are numbered one through eight. These responsibilities help to better inform published Authors of their established rights and privileges as well as details regarding their commitment to The Gold Star Journal.
Author Contract and Agreement Form
Complete the Author Contract and Agreement Form.
The contract will open in DocuSign. On the first page, fill-in your name and email address. Click the Begin Signing button at the bottom of the page and then the Continue button. The actual form will then open.
On the last page of the contract, provide your signature, fill-in your name, date the contract, provide your personal phone number, and your Citadel email address.
Download the completed form to your computer. Please name the video consent form with your first and last name followed by the words authorcontract.
Purpose of Video Consent and Release Form
Since pictures will be taken at The Gold Star Journal Academic Conference and the event will be recorded, Authors must sign the Video Consent and Release Form.

Complete the Video Consent Form.
The Consent form will open in DocuSign. On the first page, fill-in your name and email address. Click the Begin Signing button at the bottom of the page and then the Continue button. The actual form will then open.
On the first page of the contract, provide your signature, fill-in your name, date the consent form, provide your home address with the city, state, and zip code. Then provide your personal phone number and your parent/ guardian signature if under the age of eighteen.
Download the completed form to your computer. Please name the video consent form with your first and last name followed by the words videoform.
